The form simply allows our team to review your information, determine the most appropriate appointment type, and contact you with options before confirming any booking.

Review and Appointment Matching
Our team reviews your request to confirm the most appropriate appointment type and ensure the right fit for your situation. You’ll receive an email once your appointment type and eligibility are confirmed.

Book Your Appointment Online
Once confirmed, you’ll be invited to schedule your consultation directly through our online booking system.

Complete the Pre-Appointment Questionnaire
Before your session, you’ll receive a brief questionnaire to help us prepare and make the most of your time together.

Submit Test Results (if applicable)
If your appointment involves reviewing existing genetic test results, please upload them securely before your consultation.
